Imaging That Accelerates Approvals

Insurance approvals don’t begin with the claim form — they begin with the image. Incomplete or unclear radiographs remain one of the leading causes of claim delays and resubmissions. As insurance carriers increase scrutiny, diagnostic-quality imaging that clearly supports medical necessity is essential —  particularly for prosthodontic, periodontal, endodontic, and surgical procedures. Distorted, poorly positioned, or poorly exposed X-rays result in difficult-to-interpret radiographs often prompt additional information requests, slowing reimbursement and adding administrative strain. Precision at the point of image capture directly influences both treatment acceptance and the overall revenue cycle. Guidance from the American Dental Association reinforces that claims must include documentation supporting diagnosis and treatment planning. The most effective way to meet that standard is by improving imaging standards before submission. An effective imaging protocol helps prevent disruptions by ensuring: Accurate positioning on first capture, Diagnostic quality with minimal retakes, Standardized documentation across the team, Faster case presentation and claim submission.

 

The following four workflow principles can help strengthen radiographic documentation and improve approval outcomes:

Intentional Annotation

When clarification is needed, use your imaging software to highlight key findings before submission to third party insurance. Annotate your images by marking areas of caries, bone loss, or periapical pathology with clear graphics and or text. Clear visual information improves reviewer comprehension and prevent unnecessary claim delays.

Standardize Submissions

Create a simple internal checklist: correct image orientation, optimal image contrast, complete anatomic coverage, and accurate patient identifiers. Consistency in image diagnostic quality and formatting improves approval timelines and protects team efficiency.

Capture with Purpose

Insurance carriers evaluate diagnostic clarity. Ensure proper geometry, open contacts for restorative cases, and full apex elucidation for endodontic claims. A diagnostic X-ray image strengthens clinical treatment recommendation and minimizes resubmission requests.

Optimize Your Software

Present your image with a full dynamic range, proper contrast, magnify if needed to clarify pathology, and use measurement tools when appropriate. Proper exposure and geometry can make clinical findings more evident — and that clarity often determines whether a claim moves forward without delay.

Mastering Vertical Bitewings 

Vertical bitewings are essential for accurately assessing moderate to severe periodontal bone loss levels and supporting SRP case acceptance. While horizontal bitewings remain routine, vertical orientation provides extended visualization of the alveolar crest, improving assessment of attachment loss and disease progression. Clear, intentional positioning from the outset strengthens documentation, reduces retakes, and enhances clinical communication.

 

The following technique refinements help optimize first-capture accuracy:

Maximize Crestal Capture

Use vertical orientation when bone loss exceeds 4mm or periodontal involvement is indicated. This provides accurate visualization of the alveolar crest, allowing for relative measurement of crestal bone height over time. Such coverage is vital for tracking disease progression and justifying SRP therapy through clear, longitudinal documentation.

Control Horizontal Angulation

Ensure interproximal contacts are open before exposure. High-visibility white sensor surfaces aid in confirming that the dental anatomy is centered before positioning the tube head. Proper angular alignment prevents overlapping, which otherwise compromises caries detection and accurate bone height evaluation.

Midline Positioning

Position the sensor parallel to the long axis of the teeth as anatomy allows, the palatal vault is highest at the patient’s midline, avoid placing the sensor directly against the teeth. Ask the patient to relax and slowly close while displacing the tongue toward the midline to ensure full seating and minimize floor-of-mouth interference.

Adjust for Anatomical Challenges

Shallow palates or limited tolerance may prevent ideal positioning. In these cases, slightly modify vertical angulation to maintain crestal visibility or employ the bisecting angle technique for periapical views. If a Size-2 sensor is not tolerated, consider using a Size-1 sensor and capturing additional images to preserve diagnostic value.
